SUPPORTED LIVING

Living Purpose will provide individualized support and services that reflect functional and meaningful activities relevant to the individual consumer’s age, preferences, culture, and environment. Once identified, priority needs for support services and skill building are reflected and agreed upon by the individual consumer and the planning team. This includes developing and documenting methods and procedures which includes identifying and incorporating into the plan other community members, family and friends who will support the well-being of the client and their efforts to accomplish the agreed goals. These methods and procedures will serve as a point of reference and guide for the Case Managers and Support Specialists in the delivery of individualized supported living services.

 

The range of supported living services and supports available include, but are not limited to:

  • Assistance in finding, modifying and maintaining a home of their choice
  • Facilitating circles of support to encourage the development of unpaid and natural supports in the community
  • Advocacy and self-advocacy facilitation
  • Development and provision of 24-hour emergency response systems
  • Securing and maintaining adaptive equipment and supplies;
  • Recruiting, training, and hiring individuals to provide personal care and other assistance, including in-home supportive services workers
  • Providing respite and emergency relief for personal care attendants and facilitating community participation

INDEPENDENT LIVING

Living Purpose will individuals on his/her rights, to exercise choice and control over his or her life, and to safely live on their own. The agency will accomplish this by facilitating opportunities for the individual to participate and become valued members of the community. The agency will support and respect each individual’s choice and control, to access community and generic resources, to develop personal support networks and to facilitate safe and healthy lifestyles. Living Purpose will strive to identify and build upon an individual’s strengths and interests, as well as those specific services and supports which are needed for optimum levels of interdependence.
